(Individuals hired as a CTSW prior to May 11, 2017 will be grandfathered to the CTSW position with BSW, at the entity they were employed at on May 11, 2017) Three (3) years in hospital/medical social work is highly preferred One (1) year of discharge planning/care management is preferred LMSW - Licensed Master Social Worker or LCSW - Licensed Clinical Social Worker is required upon hire. CPR - Cardiopulmonary Resuscitation is required upon hire. ACM - Accredited Case Manager is preferred upon hire or CCM - Certified Case Manager is preferred upon hire Other ANCC certifications also preferred*What You Will Do* Facilitate care conferences for complex transitions, placement and palliative care needs. Serve as a point of contact for all identified stakeholders. Proactively identify patients who no longer meet continued stay criteria and communicates with the physician team. Schedule PCP, specialist or clinic follow up appointments for patients.
