PRN DAYS (as needed) covering at least 1 weekend every 6 weeks and
1 holiday a year. Some weekday coverage available* At Texas Health
Azle Texas Health Harris Methodist Hospital Azle is a
community-based hospital serving the suburban areas of Azle, Lake
Worth, Springtown and the surrounding areas. Located about 20 miles
northwest of downtown Fort Worth, Texas Health Azle provides
numerous services, including emergency medicine, surgical care,
cardiology, wound care, pain management, radiology, rehabilitation
services and an adult intensive care unit. Texas Health Azle is a
Level IV trauma center, accredited in Chest Pain Management with
distinction by the Joint Commission, and a 2013 National 100 Top
Hospital by Truven Health Analytics, which makes our facility

aspirations.*Education:*Master's Degree Speech Language Pathology
RequiredOrOther Post-Baccalaureate degree recognized by the
American Speech-Language-Hearing Association Required*Experience:
*1 year acute care experience Preferred*Licenses and
Certifications:*SLP - Speech Language Pathology Texas Speech
Language Pathology License Upon Hire RequiredOrSLP-I - Speech

Duties:*Performs age appropriate patient assessment and establishes
plan of careObtains/verifies physician order for therapy and
validates for appropriatenessReviews available medical records and
obtains pertinent medical history from patient or familyEnsures
that patient has given informed consent to treatmentEvaluates each
patient using age and diagnosis appropriate assessment tests and
measurements, including appropriate outcome assessment
tool(s)Integrates assessment results to identify functional
problems and prioritizes care needs with patient and familySets
appropriate, measurable, functional goals with time frames to
address the identified problems including patient/family goals and
preferencesDetermines plan for care, and/or appropriate therapeutic
interventions to achieve stated goalsInitiates therapy discharge
planning in collaboration with care team and patient based on
prognosis and expected discharge dispositionDemonstrates competence
in the performance of age appropriate patient assessment and
individualizes care based on age specific needs.Performs age
appropriate patient treatmentUses therapeutic treatment techniques
appropriate to patient age and desire for treatmentPerforms
treatment procedures that are within the scope of the professional
licensePerforms/delegates technical procedures appropriately and
safelyUtilizes a variety of treatment techniques to achieve goals,
if necessaryDetermines patient response to treatment and evaluates

Medicare, and other applicable regulatory guidelinesObtains
physician signature on all plans care and re-certifications,
providing proof of all attempts to obtain signature such as
documenting contacts in medical record and retaining fax cover
lettersAccurately submits patient charges and records procedures
according to departmental policyDocuments assessment of education
needs, education provided, and patient understanding of education
provided in accordance with departmental, TJC, Medicare, and other
applicable regulatory guidelinesCompletes discharge summary for
each patient in accordance with departmental, TJC, Medicare, and
other applicable regulatory guidelinesCompletes all documentation
within established time frameIf appropriate, completes all required
team documentation including intake process, plan of care, and
discharge summaryAs appropriate / required, completes required
outcome tools and any additional documentation for outpatients,
acute stroke patients and for specialty units/programs (i.e.
Lysholme, MDS, DRS, team staffing reports, etc.) ?? Documents and
reports unusual occurrences, on the job injuries, and medical
