Your responsibilities include patient assessment, recognizing health issues, creating care plans, and assessing patient responses. /n What You'll Do /n /n - You, as an RN, will conduct detailed clinical assessments, prioritize patient and family needs, create, implement, and evaluate care plans. Adjust plans to reach desired outcomes. /n - Building relationships, managing discomfort, staying professional are vital in therapeutic connections with patients and families. /n - Creating care plans reflecting values aids in resolving issues. /n - You will help educate patients, families, nurses, and community members to empower them with knowledge for better healthcare choices. Assess and document learning needs regularly. /n - Work with colleagues and community to create a care plan. Delegate tasks carefully and follow up promptly. /n - You'll use different strategies for problem-solving with patients, families, and staff. Recognize limitations and prioritize safety, effectiveness, and efficiency in patient care planning and delivery. /n - Achieve goals through teamwork, recognize contributions, commit to growth. Outdated Med/Surg/Tele RN experience or experience in LTAC, Rehab, Hospice/Palliative, Home Health, SNF, etc., will not be considered. /n Education: BSN or higher /n License/Certification: /n o RN- Registered Nurse /n o BLS- Basic Life Support (required within 30 days of hire)
