Job Description The Heart Hospital Plano- Make YOUR move! Shift- Full-Time Days- Mon-Fri. Typical schedule is Mon-Fri, approx. 8:00-4:30. The role requires flexibility to support clinic operations and program activities. The role supports physicians clinic on Mondays, participates in a weekly case conference on Fridays at 7:00 AM and provides support for NP clinic operations and patient testing Requirements- Min of 3 years of clinical nurse experience Preference- Experience in cardiology, vascular, or specialty clinic environment and familiarity with aortic aneurysm disease and management of adult patients is strongly preferred The Heart Hospital Outpatient Services Are you looking to increase your knowledge base? Job Summary The Nurse Navigator ensures prompt patient care and is the main entry point. They collaborate with the healthcare team to meet patient needs, organize care, and review requirements. The successful applicant will work autonomously and with others to advocate, educate, and coordinate for patients. Good communication skills are required. Essential Functions of the Role - You'll coordinate with health professionals to conduct necessary tests for clear diagnoses. Ensure patients have smooth access to top-quality healthcare at all treatment stages. - Collaborating with doctors and providers is vital. Education for patients and families is essential. Include diagnoses, treatments, and services. Your role is crucial in patient care. - Your role will require an exemplary level of responsibility including decision-making and delegation to ensure safe, professional patient care. - Regularly visit physicians and staff. You will be a liaison between them and patients. Provide information about our facilities and general guidance to patients and families. - You may need to help patients and families navigate the hospital and provide regular follow-ups. Key Success Factors - Communicate effectively, both orally and in writing - Organise tasks and work efficiently, as it's vital in evaluating the needs of patients and executing care plans. - Navigate through the complexity of the healthcare system, acting as a go-between and advocate for patients throughout their healthcare journey. Qualifications - Bachelor degree. - You should have at least (3) three years of relevant experience. - License Registered Nurse (RN) - (BLS) Basic Life Support should be acquired within (30) thirty days of being hired.
